How To Become A Volunteer Donor
All you need to do is fill out a consent form and give a mouth swab. The easiest way to do this is to look at our drive calendar and show up for a drive. If that is not convenient or you do not have a drive scheduled in your area, you can fill out a form online and wait for a swab kit arrive in the mail. You can also print the form below, fill it out and mail it to the registry.
Once your kit arrives, you take your mouth swab -- rub four Q-Tips on the inside of your cheeks -- seal them in the envelope provided in the kit, and mail them back to the registry.
